INSTRUCTIONS FOR SCHOOL:

FILLING OUT BOOK ORDER FORM FOR PARENTS

1) On Master Form fill out “Return checks and order form by date line.” Fill out “Make Checks payable to line.” Fill out the “Cost” for each book line (Costs will vary depending on where you decided to order from and whether you plan to pass discounts along to parents.). Parents will fill out the rest of the form.

2) Make copies of the filled-out master copy for teachers to give to all the students to take home. Once the forms are returned, the school will know exactly how many books to order.

3) Order a few extra books. (It’s good to order some extra copies for parent pop-ins on Author’s Day or for kids who come in at the last moment wanting to buy.).

4) Keep copies of book order forms available so Dia can write inscriptions in the books.
